Please Note: We will issue a refund if the doctor deems you unsuitable for this service.

Once you fill in the online questionnaire, our doctors will review and refer you for COVID-19 test accordingly. If you are not suitable for the test then you will be given further advise.

Your mobile number will be required for all correspondence in relation to the test. Please make sure it is an Irish number as HSE cannot accept non Irish number for COVID-19 testing service. Please note that we have no influence over the status of your referral once it is sent further. If you need to reschedule or cancel your appointment then advise you to contact HSE directly.

Please note that you are required to self-isolate until the coronavirus test results come back.

You can only use this service for yourself or your child. Any other person needs to fill in the questionnaire themselves. If you are using this service for your child then answer the questions on their behalf.
